community. Your role as a Service Coordinator/Case Manager As a
Service Coordinator, you play a pivotal role in supporting
Oregonians with intellectual and developmental disabilities (I/DD)
through impactful person-centered planning and ongoing case
management. Your typical day will be filled with excitement as you
coordinate with individuals, families, and providers to fulfill
essential ODDS tasks, including participating a Needs Assessment
(ONA), and the development and monitoring of Individual Support
Plans (ISPs). You'll have the opportunity to visit clients in their
homes or community settings to ensure their health and safety,
address critical incidents, and facilitate their choices in
services while overcoming barriers to support access. Your
collaborative efforts with brokerages, providers, schools, and
medical teams, along with meticulous documentation will ensure that
services are compliant with state regulations and timelines. Each

communication and collaboration with individuals, families,
providers, and community partners are vital for fostering
relationships and ensuring optimal service delivery. Proficiency
with computers and case management systems, along with sound
judgment in problem-solving and crisis response, will empower you
to advocate effectively for those you support while maintaining
professional boundaries and meeting state requirements for
mandatory reporting. Knowledge and skills required for the position
are: Knowledge of ODDS programs, rules, timelines, and
documentation standards (Eligibility, ONA, ISP). Understanding of
Intellectual and Developmental Disabilities, person?centered
planning, and trauma?informed practices. Ability to conduct
interviews, gather information, complete assessments, and maintain
accurate documentation. Strong organizational, time?management, and
case management skills with the ability to meet required deadlines.
Effective communication and collaboration with individuals,
families, providers, and community partners. Ability to
problem?solve, respond to crises, and exercise sound judgment.
Proficiency with computers, case management systems, and
ODDS?related platforms. Must meet state requirements for mandatory
