POSITION SUMMARY:The Infusion /Oncology Pharmacist is a lead pharmacist for developing and directing all out-patients infusion and oncology pharmacy services. Assume tasks necessary to ensure the delivery of IV admixtures products and clinical monitoring services to the outpatient infusion and oncology patients.KEY RESPONSIBILITIESPerformed majority of the time: - Responsible for the clinical pharmacy oncology services and ambulatory care pharmacy services. - Work with the infusion services and oncology coordinators to ensure and maintain efficient customer service, resolve issues, address concerns etc. - Able to incorporate current standards of practice into outpatient and oncology practices. - Evaluate weekly "needs" by reviewing patient schedules, ensuring quality and efficient delivery of medication. - Provides clinical drug information to physicians and nursing. - Able to identify potential issues related to the delivery of out-patients infusion including law, regulations and reimbursement. - Establish dispensing guidelines for new infusion/oncology medications. - Understand the billing process that applies to the out-patient and oncology setting. - Maintain orderly documentation and update patient files and orders in Epic Beacon electronic health record. - Formulate and Implement departmental policies/procedures for IV/Infusion to standardize pharmacy processes and insure compliance with State, Federal and CMS standards. - Working knowledge of how to safely work with hazardous drugs, USP 800 - Demonstrates knowledge of sterile technique, USP 797 - Knowledge of infusion devices and clinical considerations that applies. - Monitor department technology use related to ambulatory pumps (Infusion and Oncology). Evaluate and coordinate usage. Participate in reviews of equipment acquisition. - Use cost-effective measures in the delivery of patient care - Be able to work as an inpatient pharmacist when needed. - Checks technician work carefully and accurately. - Screens medication orders for appropriateness. - Provides care appropriate to the age of patients served based on principles of growth and development and life stages. - Performs critical dosing calculations for all patients of all age groups - Supervises support personnel-directs, educates, and checks technicians work when required. - Dispenses medication accurately in a timely fashion accurately. - Understand the special considerations in compounding and stability related to the use of antineoplastics and the use of home infusion therapies. - Follows hospital medication formulary. - Maintains adequate supply of pharmaceuticals and related materials. - Tracks clinical consults via computer. - Demonstrates confidentiality regarding patient and co-worker information according to PHI level of access. - Demonstrates awareness of safety measures. - Manages time for orderly completion of additional duties. - Maintains knowledge of department dosing/monitoring protocols. - Participates in compounding of extemporaneous preparation of medication.Performed occasionally but critical to successful performance of the job: - Orders from wholesaler via computer system. - Participates in hospital educational programs. - Performs additional duties in the absence of the Pharmacy Director. - Participates in Medication Utilization Evaluations (MUE) and Quality Improvement (QI) programs. - Maintain adequate and updated resources related to infusion and oncology medications. - Work jointly with Nursing to critique, develop and institute safer medication practice. - Support special projects when requested - Develop collaborative drug therapy guidelines and safety procedures supporting the delivery of anti-neoplastics therapies and supportive medications.Decision making and budget responsibilities: - Impact limited to the employee and the wellbeing of the patient. - No budget responsibilities.JOB SPECIFICATIONS:JOB SPECIFICATIONSEducation:Required - Licensed as a Registered Pharmacist in the State of Oregon. - Oncology Pharmacy Resident, or BPS Oncology Pharmacy Certification, or 3 years experience in oncology setting.Preferred - Home infusion therapy certificateExperience:Required - Minimum three (3) years hospital pharmacy practice experience including IV admixture, and chemotherapy preparation.Preferred - Home/out-patient infusion.Licenses, Certifications and/or Registrations:Required - Licensed as a Registered Pharmacist in the State of Oregon. - BLS required within 90-days of hire. - ACLS required within 90-days of hire.Preferred - N/AJob Related Skills, Abilities and Behaviors:Required - Excellent customer service skills. - Deals with multiple demands in a fast-paced pharmacy in an accurate efficient, calm and pleasant manner. - PC and pharmacy computer skills desired, must be proficient at typing. - Presents a personal appearance and interpersonal skills that makes a good first impression. - Demonstrates knowledge of Epic computer system. - Demonstrates knowledge of sterile technique. - Follows hospital medication formulary. - Functions as a cooperative, flexible team member in a positive manner. - Consistently utilizes problem-solving techniques to resolve complaints or concerns in and out of the pharmacy department. - Must be able to organize and work efficiently on multiple projects and activities at once. - Must be able to work independently with minimal direct supervision, set priorities, make critical decisions, respond quickly and adapt to changes in workload.Preferred - Bilingual skills a plus.#Tier5Additional Posting Information: Hillsboro Medical Center believes in providing equal employment opportunities for all qualified individuals.
