POSITION SUMMARY

Position Summary

The License Practical Nurse provides quality care to patients in their homes under the direction of a registered nurse. Assists with patient care activities within the scope of license that reflect the home health care agency philosophy and standards.

KEY RESPONSIBILITIES

Performed majority of the time

  • Collects data related to current patient physical and psychological health status.
  • Implements patient care based on identified priorities.
  • Completes routine documentation within prescribed time frames and efficiently documents on individual progress notes all pertinent and meaningful observations and information per unit policy.
  • Coordinates and evaluates patient care with other members of the disciplinary team.
  • Coordinates care with case manager to ensure continuity of care.
  • Instructs the patient and caregiver in ongoing care to be provided in the nurse's absence.
  • Teaches the patient and caregivers about medically related issues, in accordance with the plan of care.
  • Reports acute medical problems and condition changes to the case manager and/or attending physician, as directed.

Performed occasionally but critical to successful performance of the job:

  • Attends organizational and community meetings when requested.
  • Participates in agency committees, staff meetings, and in-service education as requested.

Decision making and budget responsibilities:

  • Impact limited to the employee.
  • No budget responsibility.

JOB SPECIFICATIONS

Education:

Required

  • Current license as a licensed practical nurse by the Oregon State Board of Nursing.
  • A copy of your highest academic diploma or transcript.

Preferred

  • 2 year degree in a health related field.

Experience:

Required

  • Minimum one (1) year or experience in a healthcare setting.

Preferred

  • One (1) year experience in hospital medical/surgical nursing.
  • One (1) year experience in home health care or public health nursing.
  • Experience with geriatric patients.

Licenses, Certifications and/or Registrations:

Required

  • Current license as a licensed practical nurse by the Oregon State Board of Nursing.
  • Current Oregon or Washington Driver's license.
  • Current American Heart Association Healthcare Provider BLS.

Required

  • Demonstrates skills in caring for adult and geriatric patients.
  • Uses effective communication skill, written and verbal
  • Demonstrate knowledge in areas of responsibility.
  • Provides care appropriate to the age of patients served on the job.
  • Must be able to develop skills in use of technology used in home care setting.
  • Excellent documentation skills.
  • Professional personal appearance makes a good first impression.
  • Demonstrates the ability to use equipment and carry out basic nursing skills according to nursing procedures.
  • Provides care appropriate to the age of patients served (adult and geriatric) patients based on principles of growth and development and life stages.
  • Demonstrates confidentiality according to PHI regarding patient and co-worker information.
  • Follows infection control and transmission-based precaution procedures.
  • Uses effective communication skills with patients, families and co-workers in all situations.

  • Bilingual skills a plus.

Other:

Required

  • No more than:

o one (1) accident or two (2) moving violations or

o Two (2) accidents and one (1) violation or

o No accidents and three (3) violations on or off the job within the past 36 months.

o No conviction for driving under the influence of intoxicants (DUII), license suspension, auto felony convictions or other serious violations within the past 36 months.

  • Clearance of a criminal background check and sex offender check is required upon hire and every 3 years of employment as required by OAR 333-027-0064.
  • Be at least 21 years old.
