Job Description:

The Physical Therapist collaborates with the interdisciplinary team, the patient/family/caregiver, and the patient’s primary care provider to develop a holistic plan of care focused on achieving the patient/family physical therapy goals. Instructs, coaches, guides, and intervenes to support the patient and family in achieving the best possible outcomes while remaining in the communities they value. Promotes optimal health, well-being, and quality of life of home health patients, and their families by providing skilled, intermittent physical therapy in the patient’s home. Practices independently in the patient home using excellent, evidence-based practice for assessment, clinical decision making, clinical practice, and coordination of care across the continuum.

 

KEY RESPONSIBILITIES

Performed majority of the time:

·         Acts as case manager and team lead for patients requiring home health therapy services, when nursing is either not involved in the case or physical therapy is the primary patient need.

·         Assesses the patient and collaborates with the interdisciplinary team and primary care provider to develop the patient-centered plan of care.

·         Provides physical therapy treatment to the patient/family/caregiver as ordered.

·         Completes accurate clinical documentation to effectively support service provided within specified timeframes

·         Provides physical therapy to pediatric, adolescent, adult and geriatric patients based on principles of growth and development and life stages.

·         Provides instruction to the patient/caregiver and revises the treatment plan as appropriate.

·         Maintains communication regarding patients with the case manager and other disciplines via care conferences or other appropriate methods.

·         Reports acute medical problems and condition changes to the primary care provider.

·         Provides supervision to Physical Therapy Assistants, Home Health Aides, and students in the clinical setting.

·         Completes regulatory required assessments within specified timeframes.

·         Completes medication reconciliation at specified timeframes.

Performed occasionally but critical to successful performance of the job:

·         Participates in agency committees, meetings, and staff development as required.

·         Attends community meetings as requested.

Decision making and budget responsibilities:

·         Decisions limited to the employee and the patient.

·         Works within a variety of payment models to maintain responsible financial stewardship while advocating for optimal outcomes for patients.


JOB SPECIFICATIONS

Education:

Required

·         Bachelor of Science in Physical Therapy from a school accredited by the Commission on Accreditation in Physical Therapy Education

 

Preferred

·         Doctoral degree in Physical Therapy.

 

Experience:

Required

·         Minimum of two (2) years of experience in the type of vehicle being driven.

 

Preferred

·         At least two (2) years of physical therapy experience highly preferred.

·         One (1) year experience in home health or other health setting with geriatric emphasis.

 

Licenses, Certifications and/or Registrations:

Required

·         Current Oregon license in Physical Therapy.

·         Current Healthcare Provider BLS required or within 90-days of hire.

·         Current Oregon Driver’s license

 

Preferred

·         N/A

 

Job Related Skills, Abilities and Behaviors:

Required

·         Excellent customer service skills

·         Ability to work without direct supervision.

·         Provides care appropriate to the age of patients served on the job.

·         Knowledge of physical therapy assessment and treatment methods

·         Knowledge of physical therapy modalities and equipment

·         Ability to maintain effective relationships with other health care professionals

·         Possesses effective communication skills.

·         Professional appearance makes a good first impression.

·         Must be able to develop skills in use of technology used in home care setting.

·         Excellent documentation skills.

·         Demonstrates confidentiality according to PHI regarding patient and co-worker information.

·         Utilizes problem solving method to resolve complaints, concerns, or conflicts.

·         Follows infection control and transmission-based precaution procedures.

 

Preferred

·         Bilingual skills a plus.

 

Other:

Required

·         No more than:

o   One (1) accident and two (2) moving violations or

o   Two (2) accidents and one (1) moving violation or

o   No accidents and three (3) violations on or off the job within the past 36 months.

o   No conviction for driving under the influence of intoxicants (DUII), suspension, auto felony convictions or other serious violations within the past 36 months.

·         Clearance of a criminal background check and sex offender check is required every three (3) years of employment as required by OAR 333-027-0064.

·         Be at least 21 years old.
