This role is vital in delivering high-quality care tailored to patients with diverse needs including autism services, catastrophic injury support, and home rehabilitation. The ideal candidate will be an empathetic caregiver who is passionate about making a measurable difference in the lives of patients and families. Assess patient conditions during visits and chart medical records Administer medications, provide treatments, and monitor outcomes Coordinate with multidisciplinary team members to provide optimal care Educate patients and families on health care at home Manage case notes and contributions during clinical team meetings Ensure compliance with all healthcare regulations and standards Strong clinical skills in a healthcare setting Excellent verbal and written communication Proficiency in electronic medical record (EMR) systems Critical thinking and problem-solving abilities Capability to work autonomously and make informed decisions Medical Insurance Dental Insurance Vision Insurance Life Insurance Registered Nurse (RN) license CPR/BLS certification Valid driver’s license and reliable transportation
