Unlike traditional outpatient therapy, our model follows the Primary Care Behavioral Health (PCBH) approach—consultative, collaborative, and focused on brief, functional interventions that help patients improve overall health outcomes. Position Summary: The Integrated Behavioral Health Consultant is a core member of the primary care team who works collaboratively with medical providers and support staff to address patients' behavioral health needs within the primary care setting. This role is embedded in the primary care clinic and provides brief, evidence-based interventions, psychoeducation, care coordination, and consultation to support patients experiencing a wide range of behavioral, emotional, and physical health concerns. Strong communication, flexibility, and a proactive, collaborative approach are essential. Minimum Qualifications Educational Qualifications : Master's degree in Social Work, Psychology, Counseling, or a closely related field. Licensure Requirements : Current licensure as a Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or Licensed Marriage & Family Therapist (LMFT). Experience working in an integrated care setting preferred but not required. Strong clinical assessment and intervention skills with a focus on brief, solution-focused therapy. Ability to work collaboratively within a multidisciplinary team and communicate effectively with primary care providers. Comfortable with using telehealth technology for remote patient care. Flexible and adaptable to changing work environments and patient populations. Commitment to providing culturally competent and trauma-informed care. Will attend work regularly and on time, as scheduled.
