POSITION SUMMARYThe Registered Nurse for Home Health collaborates with the interdisciplinary team, the patient/family/caregiver, and the patients primary care provider to develop a holistic plan of care focused on achieving the patient/family goals. Instructs, coaches, guides, and intervenes to support the patient and family in achieving the best possible outcomes while remaining in the communities they value. Promotes optimal health, well-being, and quality of life of home health patients, and their families by providing skilled, intermittent nursing care in the patients home. Practices independently in the patient home using excellent, evidence-based practice for assessment, clinical decision making, clinical practice, and coordination of care across the continuum. - -KEY RESPONSIBILITIES Performed majority of the time: - - - - - - - - Acts as clinical case manager and team leader for patients requiring home health services. - - - - - - - - Provides supervision to Licensed Practical Nurses, Home Health Aides and students in the clinical setting. - - - - - - - - Assesses, administers, and evaluates patient care given in the home. - - - - - - - - Develops patient-centered written plans of care in collaboration with the interdisciplinary team and primary care provider. - - - - - - - - Promotes continuity of care by collaborating with the interdisciplinary team, providers, facilities, and community resources to meet the needs of the patient. - - - - - - - - Completes accurate clinical documentation to effectively support services provided. - - - - - - - - Partners with patient/family/caregivers to ensure their understanding of patient care needs. - - - - - - - - Reports acute medical problems and condition changes to the primary care provider. - - - - - - - - Completes regulatory required assessments within specified timeframes.Performed occasionally but critical to successful performance of the job: - - - - - - - - Participates in agency committees, staff meetings, and in-service education as requested. - - - - - - - - Participates in orientation of team members to the agency and their responsibilities, provides supervision to students.Decision making and budget responsibilities : - - - - - - - - Works within a variety of payment models to maintain responsible financial stewardship while advocating for optimal outcomes for patients. -JOB SPECIFICATIONSJOB SPECIFICATIONSEducation:Required - - - - - - - - Bachelor or Master of Science in Nursing. -Preferred - - - - - - - - N/A -Experience:Required - - - - - - - - N/A -Preferred - - - - - - - - One (1) year experience in hospital medical/surgical nursing. - - - - - - - - One (1) year experience in home health care or public health nursing. -Licenses, Certifications and/or Registrations:Required - - - - - - - - Current license to practice professional nursing in the state of Oregon. - - - - - - - - Valid Oregon, Washington, or multi-state military drivers license and at least four (4) years of holding a drivers license. - - - - - - - - American Heart Association Healthcare Provider BLS. -Preferred - - - - - - - - - -Job Related Skills, Abilities and Behaviors:Required - - - - - - - - Excellent customer service skills. - - - - - - - - Ability to work without direct supervision. - - - - - - - - Excellent communication skills. - - - - - - - - Knowledge of nursing principles and practices. - - - - - - - - Knowledge of nursing process and its applications to patient care. - - - - - - - - Must be able to develop skills to use the technology used in home care setting. - - - - - - - - Excellent documentation skills. - - - - - - - - Professional personal appearance makes a good first impression. - - - - - - - - Provides care appropriate to the age of patients served (adult and geriatric) patients based on principles of growth and development and life stages. - - - - - - - - Demonstrates confidentiality according to PHI regarding patient and co-worker information. - - - - - - - - Follows infection control and transmission-based precaution procedures. -Preferred - - - - - - - - Critical care, oncology, or intravenous skills. - - - - - - - - Bilingual skills a plus. -Other:Required - - - - - - - - Must have satisfactory driving records which is described as:o - - no more than one (1) accident and two (2) moving violations or o - - two (2) accidents and one (1) moving violation or o - - no accidents and three (3) moving violations within the past 36 months and o - - no conviction for driving under the influence of intoxicants (DUII), license suspension, auto felony conviction or other serious violations within the past 36 months.
