Job Description:
POSITION SUMMARYThe Registered Nurse for Home Health
collaborates with the interdisciplinary team, the
patient/family/caregiver, and the patients primary care provider to
develop a holistic plan of care focused on achieving the
patient/family goals. Instructs, coaches, guides, and intervenes to
support the patient and family in achieving the best possible
outcomes while remaining in the communities they value. Promotes
optimal health, well-being, and quality of life of home health
patients, and their families by providing skilled, intermittent
nursing care in the patients home. Practices independently in the
patient home using excellent, evidence-based practice for
assessment, clinical decision making, clinical practice, and
coordination of care across the continuum. - -KEY RESPONSIBILITIES
Performed majority of the time: - - - - - - - - Acts as clinical
case manager and team leader for patients requiring home health
services. - - - - - - - - Provides supervision to Licensed
Practical Nurses, Home Health Aides and students in the clinical
setting. - - - - - - - - Assesses, administers, and evaluates
patient care given in the home. - - - - - - - - Develops
patient-centered written plans of care in collaboration with the
interdisciplinary team and primary care provider. - - - - - - - -
Promotes continuity of care by collaborating with the
interdisciplinary team, providers, facilities, and community
resources to meet the needs of the patient. - - - - - - - -
Completes accurate clinical documentation to effectively support
services provided. - - - - - - - - Partners with
patient/family/caregivers to ensure their understanding of patient
care needs. - - - - - - - - Reports acute medical problems and
condition changes to the primary care provider. - - - - - - - -
Completes regulatory required assessments within specified
timeframes.Performed occasionally but critical to successful
performance of the job: - - - - - - - - Participates in agency
committees, staff meetings, and in-service education as requested.

agency and their responsibilities, provides supervision to

Oregon, Washington, or multi-state military drivers license and at
least four (4) years of holding a drivers license. - - - - - - - -

- - - Critical care, oncology, or intravenous skills. - - - - - - -

have satisfactory driving records which is described as:o - - no
more than one (1) accident and two (2) moving violations or o - -
two (2) accidents and one (1) moving violation or o - - no
accidents and three (3) moving violations within the past 36 months
and o - - no conviction for driving under the influence of
intoxicants (DUII), license suspension, auto felony conviction or
other serious violations within the past 36 months. Accidents with
violations, if they occur simultaneously, will be considered one
(1) violation (incident). - - - - - - - - Clearance of a criminal
background check and sex offender check is required every three (3)
years of employment as required by OAR 333-027-0064. - -
