The Case Manager engages with clients to develop a plan based on client needs and wants to make referrals as requested by the clients. The case manager assists the Social Worker with implementation, monitoring, and evaluation of success plan. The case manager provides information, referral to clients, check-in regularly with client about goals in success plan. The case manager meets with clients to develop strong connections to coordinate client services with social and health service providers. The case manager advocates for clients with social and health services and help clients navigate systems The ideal candidate will have familiarity and/or lived experience with substance use disorders and the recovery process with the criminal justice system.This position pays $42,000.00 - $56,284.00 per year. Staff are members of a union. This is a full-time non-exempt position. In addition to compensation, MPD also offers:Medical (choice of PPO or HDHP), Dental (choice of PPO Base or PPO Enhanced) and Vision - 100% employer paid for employee only coverageFlexible Spending Account (FSA) or Health Savings Account (HSA)Basic Life, STD, LTD and AD&D - 100% employer paid 401(K) - MPD will contribute 6% of your annual salary with full vesting after the completion of 5 years of servicePTO - 24 hours upon hire and accrue monthly to 80 hours and increase to 120 hours starting at 24 months10 paid holidays + 2 floating holidays (summer and winter)4 Mental Health Days per year Sick Leave - 48 hours upon hire + 48 hours after the completions of six months of employment and accrue 96 hours each service anniversaryPosition QualificationsA minimum of two (2) years of experience with case management.Familiarity with substance use disorders and the recovery process.Familiarity with the criminal justice system.Excellent verbal and written communication skills Position PreferencesACCBO certified CRM (Certified Recovery Mentor) preferred but not requiredFluent in Spanish highly preferredLived experience with the criminal justice system.Training and experience with Trauma Informed Care.Ability to pass background check to access clients and witnesses in correctional facilities (may preclude employees and applicants arrested and jailed) and to testify in court without impeachment (may preclude employees and applicants convicted of certain crimes within the prior 15 years).Maintains confidentiality of agency records.Ability to work independently and as part of a team.Work Environment:MPD fosters a work environment that is friendly, cooperative, and respectful. Work is generally sedentary, in an office setting, courthouse, field location or jail facility.
