Old Town Recovery Center is hiring a full time case manager

The Case Manager (CM) works as a member of a multi-disciplinary behavioral health care team providing integrated behavioral health services to clients seen at Old Town Recovery Center. The CM is responsible for providing case management & housing support to the behavioral health home, ensuring care that is accountable, comprehensive, proactive, well-coordinated and patient-centered.

Schedule: Monday-Friday 8-4:30pm

Salary: $ 19.96 hourly

Duties to include:

  • Provide comprehensive case management services to all clients including, but not limited to, benefit assessment, goal setting, progress monitoring, individual money management, tenant education, advocacy and referrals.
  • Participate in outreach activities necessary to offer services and engage participation in a non-coercive manner. Escort clients to meetings and appointments as necessary.
  • Participate in the development of the care plan with client & treatment team.
  • Pursue and assist clients with accessing affordable housing options for clients both inside and outside the Central City Concern system including assistance in completing appropriate forms.
  • Use advanced knowledge and skills in counseling and assessment techniques with individuals and groups including assessing for risk of danger to self & others.
  • Demonstrate ability to build & maintain relationships with community partners including but not limited to Housing Authority of Portland, Housing & Urban Development, property management companies
    Requirements:
    • Bachelor's degree in Behavioral Sciences field or a combination of at least three year's work, education, training or experience in chemical dependency and/or mental health treatment;
    • Must meet credentialing requirements as a QMHA in the state of Oregon and be registered with the appropriate board (MHACBO)
    • Experience with severe and persistent mental illness (SPMI) preferred.
    • Must meet CCC privileging requirements as required by FTCA.
    • If using a vehicle for agency use, must possess a current driver's license, access to a vehicle, qualify as an Acceptable Driver as designated in Central City Concern's Fleet Safety policy. Must pass an initial drivers training within 60 days of being an approved driver and continued recertification training. Must maintain vehicle insurance coverage of a minimum of $100,000/$300,000 personal auto liability coverage.
    • Must pass a pre-employment drug screen, TB test, and background check. This includes clearance by the DHS Background Check Unit.
